At the dental office of Dr. D’Andrea, Dr. Pantera, and Dr. Jung, our staff will strive to make you feel welcomed and right at home from the moment you step through our front door. We have made our new patient forms available to you as Adobe Acrobat files. For your convenience, we recommend printing and completing these forms at home.

Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you.

Financial Options

We accept cash, check, Visa, Master Card, Discover and American Express. You can use these methods to cover your co-pay amount at the beginning of your treatment.

Our practice participates with some Anthem Blue Cross, Delta Dental, and ConnectiCare/Benecare insurance plans. No matter what dental insurance you have, however, we’ll be happy to file your claims for you.

We know that your insurance may be essential to your dental care. That’s why we recommend that you review your insurance policy before your appointment. Many services that are necessary for your dental health are not covered under some plans.  Patients are responsible for paying deductibles, co-pays, and other balances not paid by the insurance company.

We hope that you can choose the care you need when you need it and not when you insurance company dictates it. If you need extensive or long-term care, we offer CareCredit to help you finance your treatment.
